ABSTRACT

Background. Transplant glomerulopathy (TG) is the hallmark of chronic antibody-mediated rejection (cAMR) and can occur with or without anti-HLA donor-specific antibodies (DSAs). The role of NK cells has been hypothesized in both conditions, but data are still lacking, with no available experience with urinary NK cells.

Methods. Among the total number of TG patients diagnosed between 01/01/2013 and 31/12/2022, we considered all patients with available urinary NK, stratified for DSA serostatus and semiquantitative NK score.

Urinary NK cells were tested in flow cytometry with anti-CD3 and anti-CD16+56 antibodies and normalized for urinary creatinine excretion.

Conclusion. This study aims to investigate the role of NK cells in patients with TG in particular the correlation between the presence of NK cells in urine and DSA status.

These results suggest that the search for identifiable biomarkers using a non-invasive, reliable and rapid technique, such as flow cytometry, remains a fundamental goal in diagnostics to clarify renal damage, but also to evaluate the therapeutic progress and clinical response to the treatments performed.
