During my PhD, my research primarily focused on the study of lymphoproliferative diseases, specifically High-Grade B-cell Lymphomas. Diffuse Large B-cell lymphoma (DLBCL) is a heterogeneous disease, with different biological characteristics and clinical behaviors. High-grade B-cell lymphomas (HGBL) with MYC and BCL2 and/or BCL6 rearrangement represent a separate entity with a lower response to R-CHOP like regimens and poorer outcome
These are highly heterogeneous neoplasms, and our aim was to develop a cytogenetic/molecular algorithm. This approach integrates advanced “-omics” techniques—such as Next-Generation Sequencing (NGS) and gene expression profiling (GEP)—to achieve improved risk stratification of patients.
The study was conducted on patients with a histological diagnosis of DLBCL/HGBCL, carried out from January 1, 2015, to June 30, 2021, at multiple institutions, including Candiolo Institute – IRCCS, Candiolo (TO), Azienda Ospedaliera S.Croce e Carle, Cuneo (CN), Azienda Ospedaliera S. Luigi, Orbassano (TO) and A.O.U. Città della Salute e della Scienza, Turin (TO). We enrolled 42 cases.
On these cases, we conducted a series of molecular and cytogenetic analyses, as follow:
• Fluorescent In Situ Hybridization (FISH) to detect MYC, BCL2, and BCL6 rearrangements, identify the MYC partner gene, assess MYC copy number, and screen for TP53 deletions.
• GEP to determine the cell of origin (COO).
• NGS for mutational analysis.
All these data were matched, allowing for a detailed correlation with clinical parameters. This alignment provided a comprehensive framework for interpreting clinical outcomes, enabling a deeper understanding of patient responses and revealing potential patterns and relationships that might inform future therapeutic strategies and improve patient care.
